Aspirating and filtering atherothrombotic debris during percutaneous coronary intervention.

چکیده
he coronary “no-reflow” phenomenon can occur during eperfusion of an infarct artery or after percutaneous coroary intervention (PCI) (1). It represents inadequate myoardial perfusion despite epicardial coronary artery patency nd can be caused by reperfusion injury, embolization of therosclerotic or thrombotic debris, or ischemia-induced icrovascular damage from endothelial cell injury, leukoyte plugging of capillaries, myocardial cell edema, free xygen radical generation, and microvascular spasm. Persisent no-reflow is associated with a high incidence of yocardial infarction (MI) and the risks of congestive heart ailure and death.

Thrombus aspiration in primary percutaneous coronary intervention how to manage failure.
More than 30 years ago, thrombotic occlusion of a coronary artery was identified as the pathophysiological mechanism causing myocardial infarction (1). Mechanical and/or thrombolytic reperfusion has subsequently become the standard of care for patients with ST-segment elevation myocardial infarction (STEMI). However, reperfusion of the epicardial coronary artery does not guarantee reperfusion a...
متن کاملCoronary microembolization: from bedside to bench and back to bedside.
Coronary microembolization from the erosion or rupture of a vulnerable atherosclerotic plaque occurs spontaneously in acute coronary syndromes and iatrogenically during percutaneous coronary interventions. Typical consequences of coronary microembolization are microinfarcts with an inflammatory response, contractile dysfunction, and reduced coronary reserve. Apart from transient elevations of c...
متن کاملThe activity of pregnancy-associated plasma protein A (PAPP-A) as expressed by immunohistochemistry in atherothrombotic plaques obtained by aspiration thrombectomy in patients presenting with a ST-elevation myocardial infarction: a brief communication
BACKGROUND The expression of pregnancy-associated plasma protein A (PAPP-A) was identified by immunohistochemistry (IHC) in culprit atherothrombotic plaque specimens harvested from patients admitted with ST-segment elevation myocardial infarction (STEMI). METHODS The atherothrombotic samples were collected from a consecutive cohort consisting of 20 individuals admitted with STEMI to Stavanger...
متن کاملDevices for distal protection during percutaneous coronary revascularization.
Despite early perceptions that distal embolization of atherosclerotic plaque contents was a rare event during balloon angioplasty, it has now become clear that manipulation of atherosclerotic lesions with wires, balloons, atherectomy catheters, or stents does liberate plaque debris. The tendency of this liberated debris to cause clinical manifestations depends on the amount of debris, the size ...
متن کاملAssociation of target lesion characteristics evaluated by coronary computed tomography angiography and plaque debris distal embolization during percutaneous coronary intervention.
BACKGROUND The slow-flow or no re-flow phenomenon has been associated with distal embolization, especially of plaque debris, and with unfavorable clinical outcomes. Therefore, we examined the association between the coronary computed tomography angiography (CCTA) findings of the target lesion and distal embolization during percutaneous coronary intervention (PCI). METHODS AND RESULTS: Consecuti...
